    Over 50% of adults experiencing mental health challenges report difficulty finding timely, affordable, or culturally responsive care.

    Mental health challenges directly impact employment and stability. Studies show nearly 45% of unhoused community members navigate mental health conditions without adequate peer or housing support.

  • Early Peer Intervention

    Peer-led support breaks social isolation, builds self-advocacy skills, and creates direct pathways to long-term stability before crises occur.

Our Peer Navigation & Mental Health Programs

  • Peer-Led Connection Circles

    Facilitated by Certified Community Health Workers (CCHWs) and trained peers, these circles offer safe, compassionate spaces to share lived experiences, reduce isolation, and build personal coping tools.

  • Healthcare & Resource Navigation

    Direct guidance to help community members enroll in Apple Health (Medicaid), navigate coverage options, and locate local health and wellness services.

  • Warm Handoffs to Care Providers

    We partner with local community health clinics and accredited behavioral health agencies to provide smooth, supportive referrals when clinical care or specialized treatment is requested.

  • One-on-One Peer Mentorship

    Personalized wellness mapping with a peer navigator to help individuals set goals, build self-advocacy skills, manage daily stressors, and access essential basic needs.

  • Youth & Family Navigation

    Compassionate peer support and resource connections for caregivers and family members supporting loved ones through behavioral health challenges.
